We’re accepting new patients. For questions about patient information, or to schedule an appointment, call (931) 484-5141. For immediate needs, walk-ins are welcome at our Crossville walk-in clinic.
Crossville Medical Group accepts many commercial, Medicare, TennCare, and healthcare exchange insurance plans. If your insurance coverage includes co-pays, we will request payment at the time of your visit. Be sure to inform us if your insurance coverage changes in any way. Please visit our insurance coverage page for more detailed insurance information.
During your visit, you may be asked to fill out registration forms. You may choose to pre-register by downloading, printing and completing the forms below. Just bring them to your visit. Completing the forms prior to your visit will help speed up the patient registration process.

We ask that you please arrive 10-15 minutes prior to your scheduled appointment. Please bring the following:
- Your insurance card
- Your current Photo ID
- Any medications you are currently taking in their original containers
It’s very important that you contact the office during regular office hours for all prescription refills. We are unable to refill any prescriptions after 5pm Monday through Friday, and we are unable to refill any prescriptions on weekends or holidays. Please call our triage line at (931) 484-5141.
